Please download the three attached files and submit upon registration. These include our liability waiver, concussion forms and photography waivers. Please include the player's first and laste name in the file name.

Payment terms: A non-refundable deposit of $250 is due by May 1st, 2025 . The remaining player fee is due by the first practice. Any late payments will incur a $25 late fee per payment. Payment is accepted via cash, check or venmo and an invoice will be sent to the primary email on file upon registration.
